 | MATRIX offers vision care benefits through Vision Service Plan (VSP), the nation's largest eyecare doctor network. This plan is a PPO (preferred provider organization) Plan that offers services and benefits using doctors within the PPO network or outside of the network. The plan is designed to provide for regular eye examinations and benefits toward vision care expenses, including glasses or contact lenses.
VSP also offers access to laser vision surgery through the highest quality providers at a substantially reduced cost to the employee through their new Laser VisionCare Program.
Eye examinations, spectacle lenses, and frames, after a $10 co-pay for the standard eye exam and a $25 co-pay for lenses and frames, are covered once every 12 months (from your last date of service). Though VSP offers a wide selection of frames, not all frames are covered in full.
Elective contact lenses, after the $10 co-pay for the standard eye examination, have an allowance of $120 for the contact lens evaluation, fitting and materials. Medically necessary contact lenses are covered in full. Members may choose either glasses or contacts every 12 months.
Vision coverage is only available by selecting the “Medical Package”. The package contains medical, dental, vision, and basic life coverage. Please refer to “Pricing” for rate information.
